We work closely with Ohio’s Value-Based Care (VBC) partners to achieve quality, patient-centered, and cost-effective care.

Position Summary:
The Director of Value-Based Care will lead and implement strategies that drive value-based care initiatives across THCC. This role is responsible for operationalizing VBC programs, managing key partnerships, optimizing clinical outcomes, and reducing healthcare costs. Working in collaboration with Ohio VBC partners and internal teams, the Associate Director will ensure that THCC is positioned to excel in VBC contracts, enhance quality performance, and align with state-specific VBC goals and initiatives.

Key Responsibilities:

1. Value-Based Care Strategy and Program Development

• Develop and execute value-based care strategies in alignment with THCC’s mission and Ohio-specific VBC initiatives.
• Lead the design, implementation, and evaluation of VBC programs to improve patient outcomes, enhance care quality, and reduce costs.
• Monitor and analyze healthcare industry trends, especially in Ohio’s VBC landscape, to inform decision-making and ensure THCC remains competitive and compliant. 
• Serve as subject matter expert and internal resource for all things related to value-based care and collaborate with clinical and market leadership to guide program success

2. Performance Improvement and Quality Management

• Oversee the achievement of quality and performance metrics across VBC programs, identifying areas for improvement and implementing strategies to address gaps in care.
• Collaborate with the Chief Medical Officer, Chief Operating Officer, Director of Quality, Director of Nursing, and clinical teams to ensure VBC goals are integrated across all service lines, including primary care, behavioral health, and dental.
• Utilize data analytics and reporting tools to track patient outcomes, quality metrics, and financial performance, ensuring THCC meets or exceeds Ohio VBC standards.
• Prepare and participate in monthly operation reviews with senior leadership
• Work collaboratively with EHR team to enhance end user experience, maximize captured data, and develop skills to become subject matter expert in VBC-EHR. 

3. Partnership and Relationship Management

• Serve as the primary liaison for Ohio VBC partners, including payers, healthcare networks, and community stakeholders, to strengthen THCC’s relationships and align goals.
• Engage with statewide initiatives, such as Medicaid managed care organizations (MCOs) and accountable care organizations (ACOs), to support THCC’s objectives and drive improved outcomes for Ohio’s patient populations.
• Represent THCC at Ohio-specific VBC forums, conferences, and meetings to advocate for the organization’s interests and promote collaboration. 
• Lead and collaborate with THCC Board Committees and Clinical Leaders to meet performance targets.

4. Financial and Contractual Oversight

• Work with finance and operations teams to manage VBC contract performance, ensuring compliance with reimbursement requirements and maximizing shared savings.
• Oversee financial performance analyses, identifying opportunities for cost savings and revenue optimization.
• Collaborate with OACHC and THCC leadership to support contract negotiations with Ohio-based VBC payers, establishing realistic benchmarks, risk adjustment targets, and incentives. 

5. Team Leadership and Training

• Provide leadership, training, and development to clinical and administrative staff on VBC principles, best practices, and data-driven performance improvement.
• Foster a culture of accountability and patient-centered care within the team, emphasizing the importance of VBC in achieving THCC’s mission.
• Collaborate with HR and Department leaders to support staff education initiatives focused on Ohio-specific healthcare regulations, compliance, and value-based care expectations.
• Prepare and participate in monthly reviews with senior leadership and other forums as requested. 

 

Qualifications:

• Master’s degree in healthcare administration, public health, business administration, nursing, or a related field (or equivalent experience).
• Minimum of 5 years of experience in healthcare operations, value-based care, preferably within an FQHC or community health setting.
• Demonstrated knowledge of Ohio’s VBC landscape, including Medicaid MCOs, ACOs, and state regulations.
• Proven track record of implementing successful value-based care programs with measurable improvements in quality, cost, and patient outcomes.
• Strong analytical skills, with experience in data analysis, quality reporting, and healthcare financial management.
• Excellent communication, collaboration, and relationship-building skills, particularly with Ohio VBC partners and community stakeholders.
• Commitment to health equity and an understanding of the social determinants of health.

 

Additional Requirements:

• Ability to travel locally within Ohio as needed.
• Flexibility to work extended hours to meet project or operational deadlines.
